  • In addition to our expert doctors, nurses, social workers, therapists, and spiritual counselors, we have a wonderful team of dedicated hospice volunteers with a variety of expertise who are here to provide additional support to patients and their families. These supports are patient-centered and focus on you or your loved one's hobbies, interests, and what brings peace and comfort.

  • Our Hospice team is available 24/7 to provide both medical and emotional support, answer questions, and offer guidance. When you are part of our team, you are never alone. 

  • We provide Hospice care wherever you call home. This includes independent living facilities, assisted living facilities, or a house that you call home. 

  • Hospice care is billed through both Medicaid and Medicare, with no out-of-pocket expenses for the patient.

Our Hospice Care focuses not only on the physical, but also the emotional and spiritual well-being of our patient and their family members. We help our patients and their family members make the most of their remaining time together. We do this by providing compassionate, comprehensive care that focuses on our patient’s comfort and quality of life.

Hospice Care Nurse

Our Hospice Team is Comprised of Dedicated and Compassionate People

Our highly trained Hospice Team lives and works in this community, and we are dedicated and honored to serve our neighbors.

Hospice Medical Director

Our Medical Director provides guidance and direction for our Hospice Team.  She works with your existing doctors and nurses to ensure seamless support.

Registered Nurses

We have a skilled Nursing Team of Registered Nurses (RN) and Licensed Practical Nurses (LPN). A dedicated, skilled nurse will coordinate all of your medical care and supportive needs. Our nurses will provide ongoing support and education to the hospice patients’ caregiver(s).

Certified Nursing Assistants (CNA)

Our Certified Nursing Assistants provide personal care under the supervision of our nursing team. They are assigned tasks based on the identified personal care of our hospice patient.

Medical Social Worker

Social Workers are available to discuss the emotional challenges of serious illness and help in providing ongoing support and education.

Volunteers

Our team of volunteers provides a wide range of services to support our patients and their families.  They are caring and compassionate members of the community who are supervised and receive in-depth training and ongoing support.

Spiritual Care Chaplains

Our Chaplains provide care, regardless of your denomination.  They can help talk you through issues regarding fear, loss, forgiveness, and anger or answer spiritual questions.  They can also help in coordinating additional, specific spiritual support if desired.

Bereavement Counselor

Our bereavement services include support groups, one-on-one visits, letter mailings, and phone calls to families and caregivers experiencing a loss.  This support is provided for 13 months following the loss of a loved one.

Music Therapist

Our Music Therapist clinically uses music to accomplish individualized goals such as reducing stress and improving mood and self-expression.

Certified Wound Specialist

Our Wound Specialist will work in conjunction with our local wound care centers to ensure quality healing at home.
